Enrollment Contract

Admission:

To be accepted into the CNA BootCamp of CT Certified Nursing Assistant Training Program, the prospective candidate would be at least 16 yrs of age and have
1. (2) Forms of ID (ie.) Photo ID, Passport, Valid Social Security card, etc.)
2. (If over 18 yrs of age) Proof of High School Diploma / GED
3. Completed Medical Form provided by CNA Bootcamp of CT LLC Certified Nursing Assistant Training Program
4. Scrubs (provided by student) Black pants, Hunter Green Top

Day Training Program: (4) week training program – 150 clock hours Monday – Friday: 8:30a – 4:30p (30 minute break) Broken down: (2) week classroom training (75hrs) / (2) *week clinical rotation (75hrs)

Evening Training Program: (5) week training program – 102.5 clock hours Monday-Wednesday: 5:30p – 9:30p Saturday: 8:00a – 5p (30 minute break) Broken down: (3) week classroom training (61.5hrs) / (2) *week clinical rotation (41hrs)

*Classroom/Skill Lab: 1800sqft, main level open reception/classroom/skill lab setting; seating capacity of 12 students; resource center within skill lab along w/full bathroom.

(Class Schedule cont’d) * Barriers – Clinical Site: • Physical limitations may be a barrier to participate at clinical site. • Proof of Vaccination may be required when participating at clinical sites. • A criminal record(s) maybe a barrier to participating at a clinical site.

Barriers - Employment: • A criminal record(s) maybe a barrier to employment. • Physical limitations may be a barrier to employment. • Not having proof of vaccination(s) maybe a barrier to employment.

Important! Please note: This training program DOES NOT guarantee passing of the training program – it is designed to prepare you to take the state exam. While the school may assist with employment opportunities, the school DOES NOT guarantee employment placement.

Definitions: In this Agreement, the words “we”, “our”, and “us” refer to CNA BootCamp of CT LLC Certified Nursing Assistant Training Program. The words “you’ and “yours” refers to the student. The word “pay” refers to “pay by credit card, bank check, money order”.

Refund Policy

There will be NO refunds issued if the student: 1. Withdraws from the program on or after their start date 2. Fails the CNA Bootcamp of CT Certified Nursing Assistant Training Program 3. Is terminated from program due to misconduct (Please refer to Student Code of Conduct section in the CNA Bootcamp of CT Certified Nursing Assistant Training Program’s Student Catalog)

Withdrawal Policy
If you NEED to withdraw from the CNA Bootcamp of CT LLC Certified Nursing Assistant Training Program for ANY reason, please notify the School Director by email ([email protected]) within (3) three calendar days after the Student Enrollment Contract Agreement has been signed/accepted AND prior to beginning classes. All monies except the Registration Fee ($400) will be refunded to the student/sponsor within (30) days of Director receiving the email to cancel the training program.

Termination Policy
We reserve the right to dismiss you on any of the following grounds: 1.) Failure to pay tuition according to agreed schedule of payment as stated in this document (CNA Bootcamp of CT LLC Certified Nursing Assistant Training Program’s Student Enrollment Conract/Agreement) 2.) Unsatisfactory academic progress (69% or below “F”) 3.) Failure to maintain a record of attending at least 90% of the delivered hours of instruction 4.) Failure to comply with CNA Bootcamp of CT LLC Certified Nursing Assistant Training Program’s “Student Code of Conduct” found in the Student Catalog.

Medical Form Please complete Medical Form provided by CNA Bootcamp of CT LLC Certified Nursing Assistant Training Program.

Graduation
§ Day Training Program: Graduate will be provided with a certificate of completion (150hrs) from CNA Bootcamp of CT Certified Nursing Assistant Training Program.
§ Evening Training Program: Graduate will be provided with a certificate of completion (102.5hrs) from CNA Bootcamp of CT Certified Nursing Assistant Training Program.
§ While the school may assist with employment opportunities, the school DOES NOT guarantee employment placement.

Acknowledgments

By signing below, you and your parent or guardian (required if under age or a minor at time of signing) agree you had the opportunity to read, understand and have completed the terms and conditions stated in this document, CNA Bootcamp of CT LLC Certified Nursing Assistant Training Program’s Student Enrollment Contract/Agreement. Each person who signs below also acknowledges that they have received the current CNA Bootcamp of CT LLC Certified Nursing Assistant Training Program’s Student Catalog.
